Medical Advice & Treatment

● The diagnosis, medical advice, and treatment plans provided are tailored specifically to each patient’s condition.

● Do not apply advice or medications prescribed to you to anyone else.

● Patients must follow all prescribed instructions, wound care guidelines, and follow-up schedules.

● The Center is not responsible for any complications resulting from failure to follow medical advice or post-treatment care instructions.

Appointments & Cancellations

● Appointments may be scheduled by phone, in person, or online (where available).

● Please notify us at least 24 hours in advance if you need to cancel or reschedule an appointment

● Repeated missed appointments may result in rescheduling restrictions or fees.

Payments & Billing

● All fees for consultations, procedures, and treatments must be paid according to the clinic’s billing policy.

● We accept credit/debit cards, checks, and approved insurance plans.

● Patients using insurance must provide valid and updated insurance information prior to treatment.

● Co-pays and uncovered services must be paid at the time of service.

Refund Policy

● Consultation fees are non-refundable.

● Deposits or prepayments for procedures may be refundable only if the procedure is cancelled before any clinical preparation begins.

● Refunds, where applicable, will be processed according to our internal billing policy.

Patient Privacy & Records

We adhere strictly to the Health Insurance Portability and Accountability Act (HIPAA) and applicable state privacy laws.
Your medical records are confidential and will not be disclosed without your written consent, except when required by law or in emergencies.

You may request access to your medical records by contacting our administrative office.
A processing fee may apply for copies of records.

Limitation of Liability

● However, treatment outcomes may vary depending on the patient’s overall health, age, and compliance.

● The Center for Advanced Wound Care and its staff shall not be held liable for unforeseen complications, infections, or results outside our reasonable control.

The Center for Advanced Wound Care reserves the right to modify or update these Terms and Conditions at any time without prior notice.
